By completing this application form I understand and agree to the Terms and Conditions
listed below:

1.

When the computer is delivered it must be left unopened until the LWDP representative
calls to set it up (opening and or setting up the computer will invalidate the warranty).

2.
|
The equipment must be insured by the organisation against loss.
|
|
3.
|
The equipment must be made available for the use of all staff.
|
|
4.
|
Internet connection must be provided.*
|
|
5.
|
A key member of staff must attend a training session provided free of charge by
LCC/LWDP.
|
|
6.
|
Periodically LCC/LWDP will send training packages which have been approved which
must be added to the system and used for all the staff.
|
|
|
|
7.
|
Feedback on the use of the system must be provided to LCC/LWDP upon request.
|
|
8.
|
The use of the computer must comply with the Acceptable Use Policy which will be
given to you by the LWDP representative.
|
|
9.
|
The computer (including items supplied with the computer) will remain the property
of the LWDP for a period of 3 years. If after 3 years the LWDP has not requested
that the computer be returned, ownership will automatically be transferred to the
organisation signing this agreement.

10.
|
It is highly recommended that anti-virus software is installed on the computer,
failure to do so may invalidate the warranty.
